When you visit Icecube Active you provide us with two types of information: personal information you knowingly choose to disclose that is collected by us and use information collected by us via our Web Site as you interact with our Web Site.
When you register with Icecube Active, you provide us with certain personal information, such as your name, clinic name, clinic logo, professional title, your email address, your telephone number, your address, and any other personal or preference information that you provide to us. Furthermore, when you establish an account with Icecube Active you will need to disclose credit card details to use the service, so that we may bill you for services rendered, such as your subscription fee and the ordering of DVDs.
When you enter Icecube Active, we collect your browser type and IP address. In addition, we store certain information from your browser using "cookies." A cookie is a piece of data stored on the user's computer tied to information about the user. We use session ID cookies to confirm that users are logged in. These cookies terminate once the user closes the browser. By default, we use a persistent cookie that stores your login ID (but not your password) to make it easier for you to login when you come back to Icecube Active. This feature can however be disabled in your browser settings.
When you use Icecube Active, you may set up your personal profile, create custom physical rehabilitation protocols and save them, enter patient details, and assign custom rehabilitation protocols to specific patients. We collect this information so that we can provide you with your custom profile every time you log on giving you easy access to all custom rehab programs you have created as well as a log of all DVD's ordered, their content and their recipient.
Icecube Active may also collect information about you from other sources, such as professional associations, trade shows and email.
By using Icecube Active, you are consenting to have your personal data transferred to and processed in the United States.
Use of Information Obtained by Icecube Active
When you register with Icecube Active, you create your own profile. Your profile information, as well as your name, clinic name, clinic logo, and email are for your reference only and will not be accessible by other users of the Icecube Active Service. We may occasionally use your name and email address to send you notifications regarding new services offered by Icecube Active that we think you may find valuable. You will have the option of notifying us if you do not want to receive these updates. Parts of the above information will also be used on the labeling of DVDs you decide to issue.
Profile information is used by Icecube Active primarily to be presented back to and edited by you when you access the service.
Icecube Active may send you service-related announcements from time to time through the general operation of the service. For instance, if when you place a DVD order we will confirm such with a follow up email.
Generally, you may opt out of such emails by altering this feature in your user profile.
Sharing Your Information and Patients' information with Third Parties
Icecube Active is about making the delivery of rehab in your practice as easy as possible as well as making you look good in your community and to your patients. Your privacy and your patient's privacy are paramount to our service. We do not provide your contact information to third party marketers without your permission. We do not under any circumstance provide your patients' details to any third party marketers. We share your information and patient information with third parties only in limited circumstances where we believe such sharing is 1) reasonably necessary to offer the service, 2) legally required or, 3) permitted by you. For example: In your profile you may set that you would like to receive product and service emails from our trusted partners and only then will we share this information with them. Without you expressing this interest in your profile setting, none of your personal information will be given out to any third party.
- We may provide information to service providers to help us bring you the services we offer. Specifically, we may use third parties to facilitate our business, such as to host the service at a co-location facility for servers, to render the custom DVDs and arrange delivery, to remove repetitive information from our user lists, to process payments for products or services, or to provide search results or links. In connection with these offerings and business operations, our service providers may have access to your personal information for use for a limited time in connection with these business activities. Where we utilize third parties for the processing of any personal information, we implement reasonable contractual and technical protections limiting the use of that information to Icecube Active specified purposes
- We may be required to disclose user information pursuant to lawful requests, such as subpoenas or court orders, or in compliance with applicable laws. We do not reveal information until we have a good faith belief that an information request by law enforcement or private litigants meets applicable legal standards. Additionally, we may share account or other information when we believe it is necessary to comply with law, to protect our interests or property, to prevent fraud or other illegal activity perpetrated through the Icecube Active service or using the Icecube Active name. This may include sharing information with other companies, lawyers, agents or government agencies.
- We let you choose to share information with marketers or electronic commerce providers through sponsored groups or other on-site offers.
- If the ownership of all or substantially all of the Icecube business, were to change, your user information may be transferred to the new owner so the service can continue operations. Changing or Removing Information
Access and control over most personal information on Icecube Active is readily available through the profile editing tools. Icecube Active users may modify any of their profile information at any time by logging into their account. Information will be updated immediately. Individuals who wish to deactivate their Icecube Active account may do so by contacting the service. Removed information may persist in backup copies for a reasonable period of time but will not be generally available to members of Icecube Active.
Should you cancel, Icecube Active will maintain a backup record of your transactions, including but not limited to the content of the DVDs ordered and a record of all recipients of DVDs through your profile.
Security
Icecube Active takes appropriate precautions to protect our users' information as well as the information of patients entered into the system. Your account information is located on a secured server behind a firewall. When you enter sensitive information (such as credit card number or your password), we encrypt that information using secure socket layer technology (SSL). Because email and instant messaging are not recognized as secure communications, we request that you not send private information to us by email or instant messaging services.
Terms of Use, Notices and Revisions
Your use of Icecube Active, and any disputes arising from it, is subject to this Privacy Policy as well as our Terms of Use and all of its dispute resolution provisions including arbitration, limitation on damages and choice of law. We reserve the right to change our Privacy Policy and our Terms of Use at any time. Non-material changes and clarifications will take effect immediately, and material changes will take effect within 30 days of their posting on this site.
